An initiation fee of forty dollars* ($40.00) along with thirty dollars ($30.00) for annual dues should accompany your application.
A $3.00 Convenience fee for all online transactions.

Once your payment is made DOWNLOAD the Associate and Honorary Membership Application below.  You are required to email your applications and purchase order number along with your Name and Contact information in the body of the email.
